What total bilirubin measures

Bilirubin is the leftover pigment created when your body recycles old red blood cells. Your liver takes it up, chemically processes it, and sends it out in bile so it can leave through your stool. A total bilirubin test adds up both the processed and unprocessed forms, which is why it reflects how well red cell breakdown and liver and bile flow are working together.

What high or low levels mean

A high total bilirubin can cause jaundice, the yellowing of your skin and the whites of your eyes, sometimes with dark urine or pale stools. Common reasons include liver disease such as hepatitis or fatty liver, a blocked bile duct, and hemolysis, where red cells break down faster than usual. A frequent and harmless cause is Gilbert syndrome, an inherited pattern that mildly raises bilirubin during fasting, illness, or stress and needs no treatment. Low total bilirubin is not considered clinically meaningful and rarely signals a problem. Follow up promptly if you notice new jaundice, or if your result is high alongside abnormal ALT, AST, or alkaline phosphatase.

What does it mean if my total bilirubin is slightly high?

A mild isolated rise is often benign and commonly reflects Gilbert syndrome, especially if your other liver enzymes are normal. Clinicians usually recheck the value and look at direct and indirect bilirubin to sort out the cause.

Should I worry about Gilbert syndrome?

No. Gilbert syndrome is a common, lifelong, and benign condition where bilirubin fluctuates with fasting or illness. It does not damage your liver and generally requires no treatment or dietary change.

Can fasting or dehydration change my result?

Yes. Skipping meals and dehydration can nudge bilirubin upward, particularly in people with Gilbert syndrome, so a single elevated reading is often repeated under steadier conditions.
